Prolotherapy
Prolotherapy is a treatment that places a “proliferant” solution into a damaged tendon or ligament. The proliferant, usually concentrated dextrose or sugar water triggers the natural healing response in the body. A repair process is activated at the site in the damaged tendon where the solution has been injected. The ligament or tendon becomes stronger which reduces pain and instability.
Why not cortisone?
Acute tendon and ligament problems may involve painful inflammation
that continues beyond the normal few days necessary for healing
to start. Unlike cortisone injections that over a period of
time have a tendency to degenerate tendons and ligaments,
dextrose solution actually starts a temporary inflammatory
response that is necessary to bring healing to the area. The
treated area normally will feel more painful for a few days.
NSAIDs or anti-inflammatory medication should be avoided for
this reason. They will tend to block the healing process.
Instead, Tylenol and ice are used while healing is initiated.
What problems can prolotherapy help?
Prolotherapy is primarily used on chronic problems that have
been present months to years and haven’t responded or have
quit responding to standard care including cortisone injection.
These include:
Common osteoarthritis of joints, also called degenerative
arthritis. Often pain is produced by the tendons and ligaments
as they attach to the bone at the joint. Almost any joint
can be treated, but typical ones include knee, ankle and feet,
hip, wrist and hand.
Poorly healed injuries such as sprains and strains that
have not resolved after a few months/years. This may include
shoulder and elbow injuries along with chronic tendonitis
such as tennis or golfer’s elbow.
Unstable joint due to ligament looseness. Shoulders that
tend to dislocate is one example. Chronic back and neck pain
especially if aggravated by prolonged positions. These require
quite a few injections and other treatment including physical
therapy, medication, manipulation and cortisone injections
are used before going to prolotherapy. Unstable sacroiliac
joints that only respond temporarily to manipulation are often
relieved by prolotherapy.

However, a recent study in the Journal of Rheumatology showed a 35 point improvement on a 100 point visual injections. In other words, a patient’s chance for improvement is much greater with the use of ultrasound.”
